This questionnaire will help us prepare an accurate estimate for your event. Any information will help - if you cannot provide the necessary information, we must have the name and phone number of someone who can answer questions about technical, crew, and scheduling requirements. Once we receive your information, we will contact you with an estimate within 5-7 business days. If you have any questions on completing the form, contact us at rentals@smdcac.org or by phone at (786) 573-5316.

Please note additional rental & tuning charges apply. The 9’ grand piano Steinway is only available in the Main Stage Theater.

Please note: There is a $150 charge to drill and fill lag holes in the floor

Please note: Only one camera is permitted for archival purpose
Please not: You must advise box office of any seats necessary for placement of a camera equipment so they can be taken out of available ticket inventory.
